Forms Creation Checklist

There may be instances where you followed the instructions for creating AEM Forms and still encountered issues. This page is designed to help boost your confidence in your forms and ensure that all necessary configurations are properly applied in compliance with the State of Illinois guidelines for AEM Forms creation.

Form File

  1. Utilize the correct forms template
  2. AEM Form-file Properties should be SET TO AGENCY-ACRONYM instead of Global
  3. Apply correct Basic Configuration Information on the Forms-Container Properties
  4. Utilize the Tool bar Submit button
  5. Add ReCAPTCHA to your Form
  6. If submitting to a SharePoint list, confirm field names, Title routing slug, and route status

Form Display Page

  1. Use the AEM Form Component under SOI Site NOT the one under General
  2. Ensure that the Set Focus on Form is UNCHECKED
  3. Ensure Form covers entire width of the page is CHECKED
  4. Add the Form Title on the AEM Form Component
  5. Ensure your forms.css has the updated code.
